Under the “Employee” tab in Self-Service Banner, there should be a link labeled “Electronic Personnel Action Form”. If a link does not appear, the user may not be authorized to create EPAFs. If there is no link, contact Human Resources to authorize the user. If a user needs help filling out the EPAF, please connect with HR.
